About your Role:
As a Pre-Licensed Therapist, you will play a vital role in delivering mental health treatment services designed to foster client independence and empower families. Your responsibilities will include providing individual and group counseling to support clients in achieving greater personal, social, educational, and vocational growth and adjustment.
Horizon Community Mental Health Adult Outpatient:
HCMH provides specialized outpatient mental health services for adults aged 18 and older who are living with serious and persistent mental illnesses that significantly impact daily functioning. Our dedicated clinical team includes compassionate therapists, medication support and services, and supportive rehabilitation coordinators, all working together to help clients on their path to recovery.
**Salary $33.00-36.00hr **

EDUCATION/LICENSING REQUIREMENTS
- Masters degree from accredited school in Social Work, Psychology, Counseling, or related field
- Must be registered as an Associate Marriage and Family Therapist, Associate Professional Clinical Counselor, or Associate Clinical Social Worker
- Proof of: valid California driver’s license and auto insurance, as well as proof of education are required
- Minimum of two years experience with mental health patients (inpatient and outpatient)
- Must have direct clinical, case management experience and knowledge regarding rehabilitation philosophy
- Strong clinical and interpersonal skills, and excellent written and verbal skills
- Intermediate PC skills required
ESSENTIAL JOB FUNCTIONS AND REQUIREMENTS
- Provide crisis intervention, consultation, and activities directly related to the intervention process.
- Deliver individual, group, and family therapy for adults and children.
- Offer occupational, educational, and other guidance to help individuals develop realistic educational and vocational plans.
- Participate in interdisciplinary treatment planning to ensure comprehensive care.
- Contribute to program development, staff in-service training, and required staff meetings.
- Perform administrative duties aligned with program needs and requirements.
- Conduct counseling or therapeutic sessions to help clients gain insight into personal challenges, set goals, and plan actions based on their interests, abilities, and needs.
- Maintain accurate documentation in medical records per company and regulatory standards.
- Evaluate data to identify underlying causes of clients’ challenges, determining the suitability of counseling or referring to other specialists or institutions when appropriate.
- Uphold HIPAA regulations, ethical standards, and professional boundaries with staff and clients.
- Follow up on treatment outcomes to assess the effectiveness and reliability of interventions.
- Meet minimum weekly service hours as specified by the supervisor or program director.
- Provide consultation and education to community organizations, schools, agencies, hospitals, and county departments as needed.
- Link clients to intensive interventions aimed at developing skills to manage mental health crises, trauma, or depression.
- Teach educational classes, such as parenting, domestic violence prevention, and anger management, and deliver on-site therapy in schools.
- Meet monthly productivity requirements of 100 hours or as directed by the supervisor/program.
- Dedicate up to 80% of time to providing therapy for individuals, groups, and families, as well as supporting parents or caregivers on- and off-site.
- Transport clients as needed to foster community connections, using a personal vehicle if required.
- Fulfill on-call or after-hours responsibilities as assigned.
- Perform other duties as assigned to support clients, the program, and the organization.
Ongoing Hiring: Rehabilitation Coordinator:
Salary $21.00-$25.00hr
SUMMARY
Provides outreach, information-referral, case management, and a full array of mental health /rehabilitation services for clients which include counseling, crisis intervention, skill training, and coordinating and monitoring the assessment, treatment planning process and follow-up for each client. Level II may act as a lead recovery coordinator providing supervision and training to other recovery staff and volunteers.
QUALIFICATIONS
AA, AS or BA degree in human services, psychology or related field and one year related experience or two years’ equivalent experience
- Proof of: valid California driver’s license and auto insurance, as well as proof of education are required record and current CPR certification
- Must have minimum one year equivalent prior case management or psychiatric treatment experience
- May be required to have knowledge of rehabilitation model
- Excellent prioritization and organization skills
- Strong interpersonal skills and good written and verbal communication skills
- Intermediate level PC skills required
- Related experience: coaching, scouts, teacher’s aide, or CPS aid
- Closely monitored with six months of training
ESSENTIAL JOB FUNCTIONS AND REQUIREMENTS
- Conduct intake assessments and complete comprehensive evaluations of each client’s life circumstances, challenges, and needs. Collaborate with the client to develop a detailed and personalized plan of care with their participation and consent.
- Coordinate services by assisting clients in implementing their care plans, facilitating access to community resources, and managing the process to ensure the effectiveness of support services, including monitoring client and provider participation.
- Provide professional support through regular contact, including home visits, to offer personal guidance, consultation, intervention, and instruction tailored to client needs.
- Transport clients as needed to foster community connections, using a personal vehicle when required.
- Meet a monthly productivity requirement of 100 hours.
- Maintain accurate daily records of activities, including service progress notes and billing forms, ensuring compliance with company policies, government standards, and Short Doyle/Medi-Cal case management requirements.
- Conduct outreach and provide information and referral services to individuals who do not require or qualify for case management services.
- Uphold HIPAA regulations, ethical standards, and professional boundaries with clients and staff.
- Offer temporary interventions to former clients on an as-needed basis.
- Build and sustain professional relationships with community agencies and service providers to enhance collaboration.
- Participate in management, staff, and client meetings as well as training activities as required.
- Perform other duties as assigned to support the program and organizational goals.
